Appointment Information
Patients are seen in the office only by appointment. At the first visit, please arrive with time to fill out initial paperwork. Or, you can
download it here, complete it, and bring it with you. Please call the office at least one day in advance if you need to reschedule an appointment. Dr. Lipkin appreciates that your time is valuable and does everything he can to minimize your wait. He asks that you respect his time, and his other patients, by being on time or giving sufficient notice of a schedule change. Likewise, please understand that some appointments will run long and he might get behind. But if it is you that needs extra time, he will be there for you.
Please be prepared to make your co-payment at the time of your office visit. Do not forget to bring your insurance card, any referral forms, lab reports, pertinent x-rays, and your completed patient forms that you downloaded.
Please call the office between 8 and 4 during the week for routine matters. Dr. Lipkin can be reached, by the same phone numbers, after hours for urgent matters. True emergencies should be directed to 911 or Englewood Community Hospital Emergency Department.
